Enrollment Requirements

Documents needed for student registration:

• Birth certificate
• Lease
• Residency affidavit
• Two (2) most recent utility bills
• Transcripts (high school)
• One (1) form of parent/guardian identification
  • Driver's License
  • State ID Card
  • Passport
  • Matrícula (consulate issued ID)

Medical Requirements

Medical documents needed for student registration (must be submitted within 30 days of your student's first day):

Completed Dental Exam

• English Dental Exam Form
  • English Dental Exam Waiver Form
• Spanish Dental Exam Form
  • Spanish Dental Exam Waiver Form

Completed State Eye Exam

• English Eye Exam Form
  • English Eye Exam Waiver Form
• Spanish Eye Exam Form
  • Spanish Eye Exam Form

Certificate of Child Health Examination (must be on the Illinois School Physical form)

• English Child Health Examination
• Spanish Child Health Examination

Please turn in all forms directly into your student's school building nurse. Failure to comply will result in our student's exclusion from school until forms have been turned in.

Round Lake Area Health and Wellness Center

Schedule an appointment at the Health and Wellness Center by calling (847) 377-8800. When scheduling, state the appointment is the appointment is at the student based health center at Round Lake High School.

If you are a brand new family to Round Lake Area Schools (RLAS), you will create an Infinite Campus Parent Portal account. This will allow you to register your student, update your information, see grades, assignments, and more!

Schools of the District do not refuse to enroll a student due to the student’s failure to present their student permanent or temporary record from a school attended previously.
