Health Requirements & Forms

Certificate of Child Health Examination (physicals) and Immunization Verification forms must be completed in their entirety for all students entering grades Preschool, Kindergarten, 6, 9,  and new to District. Forms must be completed prior to the start of school to avoid student exclusion. Please note that only an Illinois physical health form will be accepted. Students entering from out of state at all grade levels are required to have an Illinois physical.

Any student wishing to participate in district-sponsored sports (competitive or intramural) must have a physical on file. The physical must be current- less than one year old, and may be a Certificate of Child Health Examination or IHSA sports physical form.

Proof of an eye examination by an ophthalmologist must be submitted to the school no later than October 15 of the year the child is enrolling in Kindergarten and students enrolling in an IL school for the first time. 

All students in grades Kindergarten, 2, 6, and 9  must have an oral health examination performed by a licensed dentist within 18 months prior to May 15th of the current school year. Dental examinations must be completed on the State of Illinois Proof of Dental Examination form and turned in to your child’s school no later than May 15th of the current school year.

Over the past ten years, three Tuberculosis exposures have occurred at Round Lake High School in 2008, 2014, and 2017.  Each time a student tests positive for Tuberculosis, this requires specific protocols to be enacted by the Lake County Health Department, including screening all students and staff with whom the student comes in regular contact. 

The Village of Round Lake has a higher than expected incidence of Tuberculosis. In the past ten years, over 10% of active Tuberculosis cases from Lake County occurred in residents from the Village of Round Lake; of those cases in Round Lake, 20% were in students from Round Lake High School. Therefore, The Lake County Tuberculosis Authority is recommending that all students at Round Lake High School who are identified as high-risk be screened for Tuberculosis.

Students who are identified as high-risk for Tuberculosis at Round Lake High School will have the opportunity to be screened for Tuberculosis. Parents may sign consent for Tuberculosis screening in the form of a blood test or a skin test as part of the registration process for the school year in Infinite Campus beginning March 21.

A Tuberculosis Risk Factor Questionnaire/Assessment will be made available on the District’s website in April for parents to complete. Students identified as high-risk as a result of this questionnaire will have the opportunity to be screened for Tuberculosis on September 9-13 at Round Lake High School. Families can also have Tuberculosis screening conducted by their private healthcare provider.  Screening will cost $45 and insurance will be accepted. In accordance with rules adopted by the Illinois Department of Public Health, a student will be exempted from this policy’s requirements for: (1) religious or medical grounds if the student’s parents/guardians present to the Superintendent a signed Illinois Certificate of Religious Exemption form explaining the objection; (2) health examination or immunization requirements on medical grounds if a physician provides written verification; (3) eye examination requirement if the student’s parents/guardians show an undue burden or lack of access to a physician licensed to practice medicine in all of its branches who provides eye examinations or a licensed optometrist; or (4) dental examination requirement if the student’s parents/guardians show an undue burden or lack of access to a dentist.

Per ISBE, hearing screening is required annually on all children in special education, children new to the district, and teacher/parent referrals. Hearing screening is also required beginning at age 3 in all licensed daycare/preschool programs. Once a child begins school, hearing screening is required at grades K, 1, 2 and 3. If you do not want your child to be screened, please communicate this to the building nurse at the beginning of the school year.
