REGISTRATION INFORMATION
Students New to District 116
To enroll your child and start the registration process, please bring the following required items to the REGISTRATION OFFICE:
- Child's original birth certificate or passport
- Two proofs of residency:
- one legal document -- mortgage or lease
- one utility bill in your name
- State of Illinois Medical & Dental Forms (completed by the first day of school). Medical and Dental forms can be found here:

The forms are available on-line. These forms may be filled out prior to coming in person for your appointment.
The Registration Office is located at 801 Sunset Drive in Round Lake. The office hours are:
Mondays, Wednesdays and Fridays 8:30 a.m. – 11:30 a.m.
Tuesdays and Thursdays 1:00 p.m. – 3:30 p.m.

Please completely fill out all forms, and bring proof of current physical and immunization records at the time of registration.
Also Required Are: Birth Certificate, Social Security Number, and Proof of Residency (2 forms). Acceptable proof of residency includes an Illinois Driver's License, State I.D., a copy of Utility Bill, Lease or the like. Affidavit of residency may also be required.
Admission Requirements
- Beginning students may start Kindergarten if they are five (5) years old or before September 1st of the year they enter school. Parents must submit their child's Birth Certificate to verify age.
Also Required Are: Social Security Number and Proof of Residency (2 forms) - Illinois Driver's License or State I.D. and copy of Utility Bill, Lease or the like are acceptable proofs. Affidavit of residency may also be required. Students are eligible to begin 1st grade if they are six (6) on or before September 1st of the year.
- All beginning students must complete the language survey form.
- If the child does not reside with his or hers natural or adoptive parent/parents additional proof of legal custody of the child shall be required. Legal custody means:
- Custody exercised by a natural or adoptive parent with whom a pupil resides;
- Custody granted by order of a court to a person with whom the pupil resides for reason other than to have access to the District's educational programs;
- Custody exercised under statutory short-term guardianship, provided that within 60 days of the student's enrollment, a court order is entered establishing a permanent guardianship and granting custody to a person with whom the pupil resides for reason other than to have access to the District's educational programs;
- Custody exercised by an adult caretaker relative who is receiving aid under the Illinois Public Aid Code for the pupil who resides with that adult caretaker relative for the purpose other than to have access to the educational programs of the District; or
- Custody exercised by an adult who demonstrates that, in fact he or she has assumed and exercises legal responsibility for the pupil and provides the pupil with a regular fixed nighttime abode for the purpose other than to have access to the educational programs of the District.
- A person is guilty of a Class C misdemeanor if:
- that person knowingly enrolls or attempts to enroll in the schools of a school district on a tuition free basis a pupil known by the person to be a nonresident off the district, or
- that person knowingly or willfully presents to any school district any false information regarding the residency of a pupil to attend any school in that district without the payment of a nonresident tuition charge.
- Students in all grade levels must have current immunizations against Measles, Rubella, Polio, Diphtheria, Tetanus, Whooping Cough, Mumps, and Hepatitis B, and must have Lead Screening. The dates registered with school must include a minimum of month and year that vaccines were give. The vaccination day will be required when it is necessary to determine the minimum time between doses.
- The School Code of Illinois. Sec.27-8.1, requires that within one year prior to entering Kindergarten or the first grade, and upon entering the 5th and 9th grades, every pupil must have current immunizations and a physical examination performed by a physician licensed to practice medicine in all of its branches. - Students transferring into Round Lake Area Schools from out of state must have current immunizations and have a physical examination less than a year old before they will be allowed to register and start school. New students from another Illinois school must also present a transfer form. Students will not be allowed to register or begin school until all requirements are satisfied.

Frequently asked questions/information for Special Education students:
- IF YOUR CHILD WILL ATTEND A SEDOL CLASSROOM ... SEDOL will send you a packet in August with important forms and information, after you have completed the District registration.
- IF YOUR CHILD WILL ATTEND A DISTRICT SPECIAL EDUCATION CLASSROOM ... You will receive this information through the mail in August if you have completed registration.
- IF YOUR CHILD ATTENDS A PRIVATE DAY SCHOOL PROGRAM ... i.e. Stepping Stones, Camelot, Central Baptist, you MUST REGISTER IN THE DISTRICT.
