REGISTRATION INFORMATION
Students New to District 116
To enroll your child and start the registration process, please bring the following required items to the REGISTRATION OFFICE:
- Child's original birth certificate or passport
- Two proofs of residency:
- one legal document -- mortgage or lease
- one utility bill in your name
- State of Illinois Medical & Dental Forms (completed by the first day of school). Medical and Dental forms can be found here:
- Physical Exam
- Dental Exam
New Student Registration is at the REGISTRATION OFFICE. Please contact the District Registrar at 847-270-9050 for additional information regarding the registration process.
The forms are available on-line. These forms may be filled out prior to coming in person.
The Registration Office is located at 801 Sunset Drive in Round Lake. The office hours are:
Monday - Friday through September 3
8:00am - 12:30pm
2:30pm - 3:45pm
KINDERGARTEN REGISTRATION INFORMATION
- Students must turn age 5 by September 1, 2010 in order to register for the 2010-11 school year.
- Health requirements include: health, dental and vision examinations
- Three proofs of address at time of registration
- Payment of fees at time of registration ($85)
You can download and complete the following registration forms to speed up your registration process. You will need Adobe Acrobat Reader installed (a free download) on your computer.
Please completely fill out all forms, and bring proof of current physical and immunization records at the time of registration.
Also Required Are: Birth Certificate, Social Security Number, and Proof of Residency (2 forms). Acceptable proof of residency includes an Illinois Driver's License, State I.D., a copy of Utility Bill, Lease or the like. Affidavit of residency may also be required.
